Customer Service Lacks Bigtime. Ordered some forms by using the company's own form #, we even described it when ordering with female on phone. Got different form in mail under same form #. Company changed form in major ways yet uses same form # - not a variation of it like adding a number or letter after the original form number in order to signify a change. When we called to discuss the form we actually need we were told in a long-winded way that had a tone that was short and basically showcased a "I don't care attitude" that the form is no longer available - but she hadn't even taken the time to help or look, she rather just turned us away after being unfriendly and unhelpful. Form is not returnable once shrinkwrap opened either.
